Identification or a coin/medal? I think German?

I got this coin off my grandparents to see if I can find any information about it. I believe it may be german and rather old!  Weighs around 19grams and is about 38mm

The obverse is probably based on this much smaller gold coin, but the reverse is in German, not Latin.
