Summary of Jurukova's comments about this fake medallion: (full translation of the two relevant pages on request for a limited time)
There are several peculiarities in the iconography, which show the hand of a modern engraver: Diadumenian's, neck is unusually long and his hair resembles lines of barley corns. On the reverse, the scene was very likely copied from a sestertius of Domitian. Unable to see the small details on the original image, the person who made this medallion changed some of the details: the lower part of the priest ends in a point, so he seems to be hanging in the air. The figure of the lyre player is higher than that of the priest (probably the emperor in the role of the priest) holding the sacrificial animal, and this, on a genuine sacrificial scent, would be absolutely forbidden.
Apart from that, if this medallion was original, then we must find out which important event the scene represents. Some bronze medallions struck in some towns of Thrace and Moesia were struck as commemorative mementos produced in connection with an event extraordinary to the history of the town. For example Anchialos, Odessos, Tomis struck bronze medallions to commemorate the visit of Gordian III, probably in AD 242 on his way to the East; Philippopolis for the visits of Caracalla (AD 214) and later of Elagabalus (bestowal of the Neocorus title); Perinthos (visit of Septimius Severus and Neocorus title) etc. So which event caused Deultum to strike the medallion in question, i.e. how was Deultum so honoured by Macrinus that was a reason to strike a commemorative medallion with the name and image of his under-age son Diadumenian ?
During his short, one year long reign, Macrinus remained in Syria until the end of his reign, and it is hardly credible that Macrinus, during those turbulent months, would have had a reason to send his under-age son to Deultum. Even if it had been the case, his visit would not have been limited to Deultum. His presence in Thrace would certainly have been celebrated by medallions of other towns, especially from the nearby town of Anchialos.
So taking all the above into consideration, we must agree with the author* who first published the description of this medallion, who is of the opinion that it is not an authentic medallion. (*Gerassimov, "Edin Ialsiv medaljon na Deultum, 277".)
