Salus standing left, feeding serpent, who is wrapped round an altar, out of patera with right hand and holding sceptre. (sometimes a spear)

mostly with short or long scepter, sometimes a spear would that be an engravers error then?

Strack 81, pl. 1 happens to illustrate a specimen from the same rev. die as yours, with portrait left on obv. He did not notice that the "scepter" has a point, so should be a "spear".

Strack does think, however, that the usual snake on the altar has been omitted on this die. However the top of the altar protrudes more to the left than to the right. I think the oversize protrusion on the left may include the snake, which is usually small and shown emerging from the top left of the altar.

and about snake missing, its true sometimes is only his his head, but more often wrapped or coiled
