Is it a fake or heavily worn coin from an uncleaned lot? Caracalla Denarius. 206 AD. ANTONINVS PIVS AVG, laureate draped bust right / VOTA SVSCEPTA X, Caracalla standing left, togate, sacrificing from patera over altar. RSC 688.

Looks to me to be corroded and worn. The coin looks like it has been cleaned already and what remains are hard deposits, which will take some time to remove.

weight, size are OK for a severan denarius.  The obverse is hard to judge from the photo, but the reverse looks right to me.  Likely just very dirty and a bit worn.

Liquid silver cleaner (thiourea & sulfamic acid) will clean it up.
