Would you guys try to clean this or leave it as is?  It almost looks like a patina on it, but it's rough and uneven.  If clean, what's the best way to approach it?

I had one almost exactly like that and what I thought was unevenness turned out to be crumbly crud that flaked off as soon as I started cleaning it leaving the coin look like a big lump of nothing with a vague profile. As odd as it is to hear it from me, I'd leave it be. It looks good to me.

From what I gather ,  this is a silver Tetradrachm.....( disregard this if it is not )

I'd be a little more agressive on this, 

I would try and soak it in Lemon juice  for 10-15 minutes  followed by a brushing and repeat ...over   and over  again  definately would not use any other type of mechanical  cleaning.

Some of the dark crud  should come off and thin out with just  soaks  and brushing

I would probably use lemon juice on it since it is silver. Would it be possible to post a picture of the reverse?

I think you should clean it, dont use any kind of Acid, I think just make it wet and rab it with wet foil all over and put it in a glass of water for 2 houres, then under hot water Just rub it with a small towel under the hot water, and plz show it both sides after done, if the dirt on rev is much thicker I dont recommend my method, but if it is as much as the obv use my way.
