Grandfather gave me a box full of old coins, this definitely looks the oldest but I can't identify it anywhere online. Please help :)

You don't give the size or weight, but it appears to be a quattrino of Pope Clement VIII commemorating the Jubilee Year of 1600. The correct size and weight should be in the range of 21mm and 2.7g.
The Porta Santa motif is the reverse side with the date (MDC) within; the papal arms are on the obverse, which you have upside down in the picture.
