The detail of the hair and the edges of the lettering look too soft, the whole coin in general looks too soft for my likeing although I do admit it could be due to harsh cleaning.

I don't think there's harsh cleaning, but heavy corrosion, you can see the remains of black oxidized silver layer on observe, Otho's neck and the back of head.

I am strongly inclined to believe this is genuine. Based upon the photos I believe that it is a heavily corroded struck silver coin that already had some wear on it prior to the onset of a long slow process of corrosion.

How could be explained the kind of wave visible before chin and forehead ?
Could it be the movement of metal during the process of striking ? (an equivalent to flow lines ?)

I think it is genuine, but the condition is horrible. I will try to find the dies from my files this evening, just to see if it can be done.

Well, to add to the "genuine" votes, I looked at this several times, and thought it is. I have not seen that many Otho denarii though to be fully confident.

I had in my files images of one much better preserved coin minted with same dies and another one with the same reverse. In my view the obverse legends look minted, not cast, so my vote stays. Again, looking it in hand would be helpful.

I am not sure about the authenticity, but someone could cast fake a coin and then corrode it to make it look more genuine.

There's no clue at all indicates it's a cast coin.
