Isaac Comnenus of Cyprus SB 1991 ? Or may it be Theodore I of Nicaea, SB 2061 ?  ???

SBCV 2061 must be right. In the Isaac Comnenus type (SBCV 1991) the emperor holds an akakia in his right hand, and clearly that is not the case here.
