FILL OUT, PRINT AND MAIL WITH A
SELF-ADDRESSED STAMPED ENVELOPE

- Name (teachers only) 

- Email address 

- Public school name and address (do not abbreviate the school name, no home addresses) 

- Class grade (grades 6 and above only)

- Class subject (must be Roman or Byzantine history or Latin - no art, special ed. or other topics)

- Date of Lesson (submit at least one month in advance, not more than 6 months in advance)

- Number of students in the class (one coin per student, up to a limit of 30 coins)

Do not send more than one request per semester.

Fill out all blocks.

Print this Form and enclose it with your request.

Provide a self-addressed stamped envelope.  Use regular stamps.

Do NOT use mailing machine postage (machine postage is valid only from the programmed zip code and date).

If more than 5 coins are requested, send a padded jiffy type envelop with two stamps.

If more than 20 coins are requested, send a padded jiffy type envelop with three stamps.
